21 FEB. 2021 · What is a frenectomy, and when might surgical treatment for a tongue-tie or lip-tie be considered?
In this episode of Tongue and Lip Tie, I sit down with Drs. Bobbi and Robert Stanley for an in-depth conversation about frenectomies and the questions patients and clinicians should consider when evaluating surgical intervention.
We begin with the fundamentals: What are tongue-ties and lip-ties? How are they diagnosed? What symptoms can they produce, and when might those symptoms affect function or quality of life? We also discuss anterior and posterior tongue-ties, treatment at different ages, potential benefits of intervention, and circumstances in which surgery may not be appropriate.
From there, we explore the procedure itself. Drs. Stanley discuss different surgical approaches—including scissors, scalpels, electrosurgery, radiosurgery, and several types of lasers—as well as clinician training, pre-operative considerations, anatomy, surgical technique, suturing, and the challenges of determining whether a release has been adequate.
But a frenectomy doesn't begin and end with the procedure. We discuss the potential roles of lactation consultants, speech-language pathologists, myofunctional therapists, and other professionals in coordinating care, along with post-operative exercises, pain management, healing, and recovery.
We also take a closer look at potential complications and concerns, including reattachment, prolonged numbness or altered sensation, bleeding, nerve injury, dry mouth, swelling, mucoceles, ranulas, ulceration, and situations in which additional evaluation or revision may be necessary.

In this episode of the Drill Fill Bill Podcast, I interview Dr. Eric S. Studley about one of the most important—and potentially intimidating—steps in dental practice ownership: performing due diligence before purchasing a practice.
Whether you're a dental student beginning to think about ownership, an associate preparing for your first acquisition, or an experienced dentist considering purchasing another practice, this conversation explores practical considerations that can help you approach the process more thoughtfully.
Dr. Studley discusses why planning should begin well before closing and explains the importance of developing a vision for the type and location of practice you want to own, evaluating demographics, strengthening your credit, developing a business plan, and gaining meaningful professional experience.
We also discuss financing a dental practice purchase, including loan prequalification and the importance of assembling an advisory team that may include an attorney, accountant, lender, insurance advisor, broker, and other professionals.
Once you've found a potential practice, the due diligence process becomes even more important. The conversation explores potential red flags, seller financial disclosures, practice valuation, leased equipment and technology, and the importance of carefully evaluating the practice lease—including assignability, renewal provisions, escalations, and exit clauses.
The interview also examines issues that can easily be overlooked during an acquisition, including practice-management software and licensing, pending litigation, employee retention, dental insurance participation and credentialing, accounts receivable, taxes, and financial records.
Perhaps one of the most useful themes throughout the conversation is simple: ask questions and surround yourself with qualified professionals who can help you identify what you don't know. As Dr. Studley emphasizes, if you cannot comfortably ask questions of your advisors, you may have the wrong advisors.

In this episode of the Drill Fill Bill Podcast, I interview Dr. Jerry Pyser about practical dental practice management strategies that dentists can apply throughout different stages of their careers.
Drawing from more than four decades in dentistry, including private practice, community-based dentistry, practice management, and practice transitions, Dr. Pyser discusses why relationship building is one of the most important skills a dentist can develop.
We explore the role communication plays in building trust with both patients and team members, including the importance of listening, addressing concerns openly, and creating an environment in which people feel heard and understood.
The conversation also examines personality and team dynamics. Not every patient will naturally connect with every dentist or team member. Developing a practice with different personalities and interpersonal strengths can help create an environment in which patients and team members are more likely to find someone with whom they connect.
We then turn our attention to the patient experience. From lighting, color, and space to clutter and even smell, seemingly small details can influence a patient's first impression of a dental practice. Creating an inviting environment is one component of establishing trust before treatment is ever discussed.
Dr. Pyser also shares a structured five-step approach to case presentation and acceptance: highlight positive findings, identify areas of concern, discuss prognosis, review the diagnosis, and present recommended treatment. Clear, organized communication can help patients better understand their oral health, clinical findings, and the treatment being recommended.
Finally, we discuss how a dentist's priorities may change throughout a career. New dentists may benefit from becoming students of communication. Dentists entering the middle of their careers may need to transition from management toward leadership. Those who feel their practices have stalled may benefit from an outside assessment, while experienced dentists may begin exploring new professional opportunities, alternative ways to use their skills, or planning for practice transition.
